Here are the forms you'll need for your PolarExplorers expedition or training. Some expeditions have additional paperwork requirements (skills questionnaires or waivers of transportation providers, etc.) that we'll send to you upon registration.

For your financial protection, medical evacuation and trip cancellation insurance are required on all our expeditions. We can provide you with a quote and/or make recommendations for providers. If you live in the United States or Canada we recommend Travel Guard Insurance. If you live outside the USA or Canada Ripcord may be your best option, or you may have a preferred insurance provider from previous experiences. ​
